Acknowledgment of Risk and Waiver of Liability

I, the undersigned parent or legal guardian, on behalf of my child, acknowledge that participation in soccer training, camps, and related activities conducted by Pegasus Soccer LLC involves physical activity and inherent risks of injury. I understand that participation is voluntary and at my child's own risk.

Assumption of Risks

I recognize that soccer and related activities involve risks, including but not limited to physical injury, illness, death, and loss of personal property. These risks may arise from the nature of the activity, the condition of facilities and equipment, the actions or negligence of other participants, weather conditions, and lack of hydration, among others.

Waiver, Release, and Indemnification

In consideration of my child's participation in the soccer activities, I hereby waive, release, and discharge Pegasus Soccer LLC, their owners, officers, directors, agents, employees, contractors, coaches, volunteers, and representatives (collectively, the "Released Parties") from any and all liability for injury, illness, death, or damages, and from any claims, demands, or causes of action that may arise from my child's participation, whether arising from negligence or otherwise.

I agree to indemnify and hold harmless the Released Parties from any liability, claims, demands, damages, or costs, including attorney's fees, arising out of or related to my child's participation in any soccer activities.

Medical Release

I authorize Pegasus Soccer LLC and its designated representatives to seek and obtain medical treatment for my child in the event of injury or illness during participation in Pegasus Fútbol soccer activities, including necessary transportation to and from medical facilities. I agree to be responsible for any costs associated with such treatment.

Media Consent

I grant Pegasus Soccer LLC permission to photograph and/or record my child during soccer activities. I understand that such photographs, video recordings, and other media may be used for promotional materials, social media, website content, and other marketing purposes.

I waive any right to inspect or approve the finished products, including written or electronic copy, wherein my child's likeness appears. Additionally, I waive any right to royalties or other compensation arising from the use of these materials.

Acknowledgment of Understanding

I have carefully read this Waiver and Release Form. I fully understand its terms and understand that I am giving up substantial legal rights, including my right to sue the Released Parties. I acknowledge that I am signing this agreement freely and voluntarily, intending it to be a complete and unconditional release of all liability to the greatest extent permitted by law.
